I know you said you checked for pandas, but pandas can't really be ruled out with any blood test. This level of anxiety and ocd in children is almost always caused by brain inflammation. Therapy will get you limited resulrs. Did you try the ibuprophen test?

Try giving your kid motrin or aleve (I find aleve to work better for my PANS kid when shes in a flare) for at least 3 days round the clock. See how your child is doing.

I strongly believe, unless there is trauma and/or abuse. Ocd/anxiety is physiological. (Pans/pandas/ae) in children.
And even if there is/was trauma and abuse, thr stress can actually cause a child to develop autoimmune issues (which pans/pandas/ae ultimately is.)

I have a child with PANS. In order to find true, compete and lasting healing you need to treat the whole child.
My child is 5 years in and is almost symptom free. We treated medically/physically/symptoms and also emotionally (first thru body work then when she was doing better we did ot, talk therapy, skill based therapy)
She still has some lingering issues. Adhd (we ultimately put her on no stimulant meds and she bh has zero side effects and all the benefit) and mild taurettes that's getting milder and milder bh bh bh!!!

OCD is very treatable and requires therapy by an Exposure Response Prevention trained therapist. Medication can also be helpful.
Contact Relief, they can usually give you an approriate referral.

Ibuprophen test means giving your child ibuprophen (motrin or advil, preferably dye free) around the clock for 3 days and tracking their behavior. Ibuprophen is an anti inflammatory that takes down brain inflammation so if you see improvement you know that inflammation is causing the behaviors.

There really is no accurate blood test for pandas. Pandas specialists will diagnose based on symptoms and medical history. Bloodwork can give them clues though. The ibuprophen trial is telling, and how they respond to treatment.
