When I am out during the day wearing my wig I get migraines
When I’m home not covering I’m ok
If I put it on during shabbos I immediately need to take it off because I feel the incoming pain

I had this OP. I got a larger cap but it still didn't help me. I ended up getting a scarf and a shaper with velcro so I can adjust to a size that doesn't hurt me at all - and I've never looked back. I may stand out but I'm comfortable, and migraine free BH.

That doesn’t go against what I said about the root being from something else and the wig is the cherry on top.
You need to find the root of why your head is so extra sensitive

I find it super challenging. I shaved the back of my head. When I look in the mirror the front is visible so I don’t look crazy but the less hair has been super helpful

I wear a lightweight hat fall and a straw hat or wool fedora.
I also get terrible headaches from full wigs.

I got a lace top topper from Milano. They have many diff types you can try on. With more or less coverage. I have 100% coverage. I had them sew in some velvet so I don’t need any bands or clip and it’s beyond light. I still only wear it when going to my kids school or I need to be presentable but I’m not living on imatrex anymore.

I actually took my wigs I got married with that have been sitting and collecting dust to get a makeover. Cut out so many wefts , ear tabs, made the cap to be the size of a yarmulka (they call is a 3/4 wig. cut bangs and added the velvet. Also a game changer. New wig for $400

No matter how many different wigs I tried. It's the fact something is on my head gives me migraines. I wear a tichel most of the time besides for when I got to shul or a simcha. I've gotten pretty used to wearing a tichel everywhere.

I know tichels won’t help because I can’t even wear it at home
I would love to figure out a root cause but I am not sure where to start!

I sympathize with this. I get migraines really easily too from wigs and even from tichels (unless they’re quite loose pre-tieds). Have you considered Botox for your migraines? I don’t know if it will help the wig issue specifically, but it has helped my migraines a lot and has made them less frequent.

Hate to say it, but you need the more expensive wigs. The Israeli hair is light and doesn’t give headaches. The processed wigs do. That’s my experience
I only buy unprocessed Israeli wigs now, even though they start at $4k
