Has anyone found a Dr who is especially good at dealing with swallowing issues?
Has anyone been helped by a change in diet, suggested by a dietician, or anything else?

I went to a kinesiologist who recommended 2 foods to eliminate from my diet. I rarely have any reflux anymore after suffering terribly for many years. The person I went to is not working right now but I'm sure there are plenty of people who can help you with hk.
