He's in Lakewood. Truly amazing and a tremendous baal chessed too. He is very matter of fact and direct so his bedside manner may come across as a bit rough around the edges but don't get scared off as he can be tremendously helpful bezH.

Sorry don't have it on me right now. You can look it up in the Lakewood directory. It's yechezkel englard on e 7th st. The home number listed is the way to reach him as he doesn't give out cell number. Best time to call is between 7 and 8pm
