I think if you are asking in a complementary way " wow you lost so much kol hakavod what did you do? " is ok for friend and acquaintance ... for a friend just ask

Yes, it's absolutely personal. I'd freely compliment someone on how good she looks but not ask for specifics unless she volunteers.

I have a feeling it became a more inappropriate question since surgical options have become available.
Say you met an acquaintance and said 'Wow, you look great, what did you do?!' she stammers and says 'Um, I had surgery'. That would be a real ouch situation which would never be uncomfortable had she answered 'Oh, I joined Weight Watchers'.
Of course if it's a close friend and you know she was on a diet plan then there's no harm in asking which diet plan it was.
