My 4 month old suddenly the past few days has not been sleeping and is quite fussy night and day. Is this a reason to visit the dr?

Not unless she shows signs of sickness or pain such as pulling at her ear, refusing to eat, or crying in a particularly high-pitched way indicative of pain rather than disgruntlement. She may be going through a growth spurt and need more food, or she may be uncomfortably hot and sticky, like most of us in recent days.

No pulling at her ear but she has been crying in a high pitched way at times (during the night and while nursing).
When she's in my hands she seems perfectly happy and content.

I would second teething. The comfort of you holding them calms them. Nights are worse for anything, teething included. Doesn't necessarily mean she'll get teeth anytime soon.

This also crossed my mind. High pitched crying is typical response of brain inflammation after shots.
