At a loss of what to do

my 3 yr old son has been trained for a few months now, night and day. but he still refuses to make #2 in the toilet. he knows when he has to make and holds it in until he's wearing underwear. I've tried the small potty the big toilet and all sorts of rewards and consequences.
he just doesnt seem to care when and how he makes, even on the floor! as long as its not in the right place.
I'm going nuts. can anyone help me?

I wish I could help. My 1st trained quickly & nicely. My second is petrified of the potty & the toilet so I'm having a problem too.
