If she signed a form for the agency then she doesn't need to sign for specific provider.
Some agencies are contracted to specific schools based on location.

I'm a speech therapist in NY
I never start working with a child until I speak to the parent. I need to document a log of our phone conversation, but there is no actual form that is signed
I do find it strange that you are so concerned about him being pulled out at 28 months. That's really young. Most kids don't start complaining about getting pulled out until second grade or so.
