How do you express appreciation? Tell he.. write a card? You don't have to express appreciation the same way everyone else does.

what is if you just started to go to therapy a few weeks you also need to send something or just wait till next year?

I gave it to her in her hand. She thanked me and put it aside and we had a regular session.
You can hand it to her at the end as you’re leaving if that feels better or not give at all.

I work in a mental health office with social workers and a psychiatrist in the NY area.
Every Purim, clients bring mm and mostly leave at the front desk for their therapist starting Adar. It's cultural, some clients bring some don't. Many food items are put in the break room.
I see pencil holders, coffee mugs, little plaques, pens, poems, etc. in some offices too.
There is a rule that any gift of money or any gift worth more than $50 must be returned to the giver.
Oy I'll need to avoid the break room the next 5 weeks!

There are other ways to show appreciation - verbal thank you at the end of each session, giving feedback.

The problem is that when I say thanks at the end of a session it feels more like the polite thing to do rather than a real acknowledgment of appreciation
